There is one condition that is often mistaken for a hemroid, which is a perianal hematoma. This causes problems because the two conditions do not call for the same treatment.

A hemroid is a vein which has enlarged and taken some of the surrounding tissue with it.

A perianal hematoma is caused when a vein ruptures and leaks blood into the surrounding tissue.

When that surrounding tissue fills with that blood, it causes a swelling to rise usually, but not always, on the outer area of the anus.

The vein itself is not swollen, but the blood filled tissue is.

A perianal hematoma must have the blood removed, by syringe if the blood has not clotted yet or by a simple incision if it has.

When dealing with a simple perianal hematoma, the incision usually heals cleanly and without stitches, perhaps only leaving a skin tag behind if anything.

However, when a vein that is already swollen into a hemroid ruptures and leaks blood, the hemroid has become a bleeding hemroid and the whole hemroid may need to be be surgically removed.
